Is there a specific form that I need to fill out to file a parenting time
complaint? We do have a parenting time order in place and it is not
being followed. I just need to know if there is a specific form that
needs to be filled out to file the complaint or if I can just write it
down on paper and send it in.
Answer
There is no special form you need to fill out, but you must put your alleged
violation in writing. Include the specific information as it relates
to what order (date) was violated, and what the terms were. Include
how it was violated, including dates and circumstances surrounding the
alleged violation.
Identify at the beginning of your letter, or title it as a "parenting
time violation/complaint" and the appropriate unit will review your order
and complaint and respond.
